HIV/AIDS CASE MANAGEMENT AND HOME/COMMUNITY-BASED CARE SERVICES AGREEMENT AMENDMENT (1st District) (3 Votes)
IT IS RECOMMENDED THAT YOUR BOARD:
Approve and instruct the Director of Health Services, or his designee, to sign Amendment No. 3 to Agreement No. H-205189, substantially similar to Exhibit I, with AltaMed Health Services Corporation for the provision of additional HIV/AIDS case management and home/community-based care services, increasing the maximum County obligation by $44,550 for a revised total maximum County obligation of $147,550, 100% offset by County AIDS funds, effective upon Board approval, with no change to the expiration date of February 28, 1999.
PURPOSE OF RECOMMENDED ACTION:
In approving the recommended action, the Board is authorizing the Director of Health Services, or his designee, to sign an amendment with AltaMed Health Services Corporation (AltaMed) for additional HIV/AIDS case management and home/community-based care services in the East Los Angeles area to HIV infected persons.
JUSTIFICATION:
AltaMed is the only medical provider of comprehensive HIV/AIDS services, in a community-based setting, in the East Los Angeles area serving the Latino communities. Initially, the community, as well as other community-based providers were not accessing the HIV/AIDS case management and home/community-based care services from AltaMed. After an aggressive marketing campaign, a significant increase of home health services was realized and AltaMed has exceeded the number of attendant care hours projected for this contract period. As a result, they have exhausted their annual allocation.
Approval of the amendment will ensure the continued provision of HIV/AIDS case management and home/community-based care services through the end of the contract term.
FISCAL IMPACT:
The total maximum County obligation under the amendment is $147,550, including $44,550 available in FY 1998-99 County AIDS funds.

FINANCING:
The total maximum County obligation under the amendment will be increased by $44,550, for a revised total maximum County obligation of $147,550, offset by $103,000 in CARE Act Title I, Year 8 funds and $44,550 in County AIDS funds.
Payments under the amendment will continue to be reimbursed on a fee-for-service basis.
FACTS AND PROVISIONS/LEGAL REQUIREMENT:
Since 1992, the Board has approved agreements and amendments with various agencies who participate under the California Department of Health Services (CDHS) AIDS Case Management Home/Community-Based Care Project.
The contract agencies provide short-term interim in-home attendant care to persons with HIV disease or AIDS who have fully utilized their Medi-Cal waiver allowance and are awaiting admission into other Federal, State, or County programs.
Patients receiving these services are enrolled in the Medi-Cal Waiver Program and are entitled to receive in-home attendant care not to exceed the State regulated allotment. Some patients fully utilize their allotment before the end of their illness when services are most needed. Clients awaiting admission into other Federal, State, or County programs require interim care for which there are no other resources available.
The CDHS has existing agreements with local community providers including AltaMed, to provide case management home/community-based care services in Los Angeles County.
On November 14, 1995, the Board approved an agreement with AltaMed to provide HIV/AIDS case management and home/community-based care services.
On March 18, 1997 and February 24, 1998, the Board approved Amendment Nos. 1 and 2 with AltaMed for the continued provision of HIV/AIDS case management and home/community-based care services.
The Department of Health Services is recommending the amendment with AltaMed to maintain continuity in services that will allow AltaMed to continue providing attendant care to homebound HIV/AIDS clients through the end of the contract term.

County Counsel has reviewed the amendment (Exhibit I) as to form.

IMPACT ON CURRENT SERVICES (OR PROJECT):
Approval of the amendment will allow AltaMed to continue providing HIV/AIDS case management and home/community-base services to clients in the East Los Angeles area.
